Member Form Submission Guidance

Pre-Retirement Beneficiary Designation Form Submission Reminder

Pre-Retirement Beneficiary Designation Forms received by fax will be returned to the employer. Do not submit with Employee Data Form, these will be returned.
Active and Inactive NMERB Members must complete a Pre-Retirement Beneficiary Designation Form to designate a beneficiary for their benefits. Members should review their pre-retirement beneficiary designation when significant life events occur and update if necessary.

Only current forms will be accepted. Current forms are listed on the this website. Please update your organization’s intranet and any other resources that contain NMERB Member Forms.
Submit original documents via mail or electronically through DocuSign when applicable.
